You must fill in the following boxes:
Your Name (The Adult supervising the child)
Your Email Address
Session Date (The date you wish you book a session for).
The Childs Name, The Childs age (This is the Childs age on the day of the event)

Please check the Calendar Events in case we have a last minutes location change for the event

To become a new member please download the New Members application form from the link below, Print and fill in. You can then book your child in via the booking form. Please remember to bring the printed form to the event. 
We will have some paper based formed at the event but they are limited.
